Parth Shukla, M.D. is an Emergency Medicine resident physician at Morristown Medical Center in Morristown, New Jersey (U.S.A.). Neal Dharmadhikari, M.D. is a first year Internal Medicine resident at Brown University/Rhode Island Hospital in Providence, Rhode Island (U.S.A.).  The two connected during medical school to develop a product used by various healthcare students and residents in training. In October of 2015 they listed their product online through Amazon.com. They called it Perfect H&P and it had the following description:

Our flagship product is a white-coat-sized notebook designed specifically for medical professional students to carry with them while interviewing and examining patients. 

Each sheet in the notebook is a concise and thorough template for recording medical history and physical exam findings that was designed with rounding presentations in mind. This way you can shine on the wards and learn to do a proper History and Physical (H&P)!

Dr. Shukla then wrote about his and Dr. Dharmadhikari’s experiences about starting a business in medical school in the online medical student magazine in-Training. They have been able to sell over 25,000 books in nearly every  state, break even, and even create larger margins. Their larger profit margins allowed them to pay their rent, donate wholesale profits, and help fellow students.
